Communication is key, right?
We all know that; or we learned it somewhere along whatever education we went through; or heard it during a presentation at a meeting… So, we all agree that if we communicate properly, we can avoid harmful events and improve patient safety?

Then why aren’t we doing it?
A recent systematic review investigating how poor communication between health care practitioners and patients (and between different groups of practitioners) affects patient safety, reveals that in 13.2% of safety incidents, poor communication was the only identified cause. When investigating whether poor communication contributed to patient safety incidents alongside other causes, poor communication contributed to 24.0% of safety incidents… (a neat video summary of the study was published too)
Those are almost always incidents that are preventable, with good communication
